MEDICAL SCRIBING
A Medical Scribe is essentially a personal assistant to the physician who performs EHR (Electronic Health Record) documentation. A scribe compiles information about the patient, such as the patient's visit, previous medical histories, and so on.
A medical scribe's primary responsibilities include the following:
1. A scribe is paired with a single doctor for each shift.
2. Scribes accompany the doctor as he interviews and examines the patient.
3. Scribes record the doctor's medical history as it is given to the patient, including details on the patient's current health, a review of their systems, previous medical and surgical histories, social and familial backgrounds, medications, and allergies. Scribes document the physician's actions and discoveries during the physical examination.
4. Scribes record the results of laboratory and radiographic studies as directed by the doctor.
5. Scribes accurately document patient-care activities such as doctor-to-doctor interaction, family communication, and re-examination of the patient.
