                               A Medical Scribe is a person who records all interactions between a doctor and a patient in Electronic Health Records (EHRs). By conducting patient data charting in real time, medical scribes relieve doctors of their labor and allow them to focus more on patient care. The use of scribes allows for improved management of patient flow through the department as well as increased productivity to see more patients. 

                             A scribe must also be aware of the documentation difficulties, such as whether symptoms or histories should be included or excluded from the document.

                             A scribe must be knowledgeable of and trained in coding and billing regulations when filling out a patient chart. A scribe must be aware that excellent patient care is strongly tied to the scribe's good relationships with other healthcare providers such as nurses, pharmacists, paramedics, and so on. A scribe should be able to precisely document the symptoms, lab findings, values, and tests when completing a chart.
