                                     A Medical Scribe is a healthcare practitioner who documents patients and conducts secretarial tasks on behalf of a physician. Medical scribes today undertake much of their job in an electronic health record (EHR) system in healthcare facilities that have converted from the previous paper record system, however some still manually record information in patient charts. 

Among the common responsibilities and expectations are:

• Maintain the Electronic Health Record (EHR) system by entering data for each patient.
• By examining the patient's medical record and interactions, write the History of Present Illness.
• Recording the patient's answers to the physician about food and activity, as well as the required blood tests and values.
• Transformation or translation of physician dictation from Dictaphones, followed by recording in EHR.
• Writing referral letters or other sorts of correspondence for correlation.
